New Patients

The doctors welcome new patients who live within our practice area. If you move out of the practice area you may need to register with another practice.

All new patients are invited to make an appointment to have a health check with our practice nurse.

Register With Us

To register with us, please check you live within our catchment area; you can do this using the map below.

  • We also ask that new patients obtain their NHS number from their previous GP surgery for ease of registration. If you do not have an NHS Number, you can still register with us.

Once you have ensured you live within our boundary, please complete the online ‘New Patient Registration Form’.


Alternatively, you can register at the practice by reading the ‘New Patient Information Letter’, then completing the ‘Registration Form’ and ‘Health Questionnaire’.

If you complete the paper registration forms, please leave them with a member of our reception team at one of our sites or email them to colton.andthegrange@nhs.net.


Registration forms are also available from Reception at the Practice.


We would encourage patients who take regular medications to obtain a prescription from their current practice before registering; this will ensure that patients have enough medication to last them until they are able to attend a New Patient Review Appointment at our Practice. When you register with us, we will arrange for the practice pharmacist to review your current medication list and add them onto the system ready for when you are next due a prescription. Repeat prescription requests take 2 working days. 

All new patients are offered an a new patient health check using our self check kiosk in reception. 

Medical treatment is available from the date of registration using our online request form. Please contact our reception team for further information or visit the appointment page of our website.
Patients over 75 years will be informed of their named accountable GP and will be invited for a free NHS Health Check with a Health Care Assistant. 

The NHS Health Check is a new initiative to improve overall health and well-being in the community. Its aim is to improve the care and support given to the over 75’s population. This will provide assessment and screening of patients health and social care issues based on questions and measurements. Following this patients will receive personalised advice and if needed, an onward referral or signposting to appropriate support services. 

If you are already registered with the Practice and are ages 75 year or over and do not have any long-term conditions, please contact the surgery to book in for a NHS Health Check, if you have not done so already.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient. 

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Safer Surgery

We are a Safe Surgery for everyone in our practice area.

We might ask for ID or proof of address. But if you don’t have any and you live in our practice area, you can still register with us.

We won’t ask for immigration documents.

Overseas Visitors

Those who visit the UK from overseas must pay for their care when they are in England.

In 2015 changes were made to how the NHS charges overseas visitors for healthcare so the NHS does not lose out on income for the services these patients have received.

Find Your NHS Number

Sometimes when filling in online forms or speaking with our staff, you may be asked for your NHS number.

This online tool will help you find your number if you do not know it.

Please Note

You do not need to know your NHS number to use NHS services, but it can be useful to have it.

Other ways to get your NHS number

If you cannot get your NHS number online you can:

  • find it on any letter from the NHS like a prescription or appointment letter
  • call your GP surgery and ask them for your number
