We have been advised by LTHT that the GP practice cannot contact the Hospital to chase a patients referral, the patient is advised to do this themselves. 

When contacting the Referral Choose & Book Team patients are required to have their UBRN and Password at hand. This information will have been sent to you by the practice at the time of referring. If you do not have this information, you will need to contact the practice.
