Our Nursing Team includes Advanced Nurse Practitioners, Specialist Nurses, Nurse Prescribers, Practice Nurses, Nurse Associates, Trainee Nurse Associates, Health Care Assistants, Phlebotomists and Students Nurses. We have a number of nurse led services with Senior nurses who hold additional qualifications or have specialised in the care they provide. Our nursing team works closely with a team of GP’s, Pharmacists and Specialist Nurses.
Some nursing services are not part of the GP core contract but we may provide them as part of a local scheme or shared care arrangement. You may also be referred into another service or asked to attend a local clinic.

Wound Care
GP practices are not commissioned to provide wound care services, however, our nursing team are trained to assess and manage simple wounds including leg ulcer management. An interim wound care service arrangement is in place with the Leeds West Yorkshire Integrated Care Board (ICB) and some GP practices. Leeds Community Healthcare and the hospitals also provide wound care services for complex/specialist wound care. You may referred you into a citywide service for your wound care or supported to self care your wounds. If you are housebound and require wound care, you will be referred into the Neighbourhood Team if you meet their criteria.

Injections
Some injections are not commissioned in Leeds for GP practices to deliver as a core service. Our nursing team are trained and can administer some injections if they have been requested by a GP, Pharmacists or Specialist Clinic under a shared care arrangement or local service.
- B12 (Hydroxocobalamin) Injections

- Weight loss injections are not commissioned as a core GP service and is not a service available at the surgery.
Health Checks
Our Nursing Team are trained to offer a range of health checks depending on your medical conditions or age. New patients will be asked to complete a health questionnaire and use our self check pod to measure your BP, pulse, height and weight. The following health checks are available for those eligible.

Long Term Condition Reviews
Our Nursing Team are trained to manage patients living with a range of long term conditions. Some nurses have specialised in disease management and can prescribe. They also work closely with the GP and our pharmacists to manage your care and medications. Depending on your condition/s, you may be asked to attend an initial appointment to have your bloods, observations checks such as your blood pressure, pulse, urine, breathing test and weight along with gathering information on your lifestyle and medications. A follow up appointment will be arranged with a Senior Nurse, Pharmacist or GP to complete your review either in person or telephone.
Some patients maybe under the care of a specialist team to manage their long term condition. We may not invite you for a review if you have already attended their clinic and we have received information about your care and treatment.

Screening (Smear Tests)
Cervical screening, also known as a smear test, is a test offered to women and people with a cervix aged 25 to 64. You will either receive a national screening invitation or had an invite from the surgery if you are eligible and due your smear test.
We have evening and weekend appointments in addition to our clinics during normal opening hours.
Investigations
Some investigation tests are not commissioned in Leeds for GP practices to deliver. Our nursing team are trained and can carry out some investigations if they have been requested by a GP, Senior Nurse, Pharmacists or Specialist Clinic under a shared care arrangement or local scheme.
You may be referred or asked to attend a local Community Diagnostic Centre (CDC) for some investigations. Community Pharmacists also offer BP Monitoring Services.

Other reviews
Our nursing team may complete part of your annual review of HRT or Pill Checks. The information will be passed onto a GP or Pharmacists to complete your medication review. You can also attend a local pharmacy for your contraceptive pill.
