These appointments require an initial consultation with a GP, you will then be referred into our Minor Surgery Service if required. 

Community Minor Surgery Provider (E-Referral)

If you have been referred by your GP to our Community Minor Surgery Service, your GP practice should have provided you with a UBRN Number and Password. You will be asked for this information when you contact us, so please have the details available. 

Upon contact you will be added to a waiting list, once an appointment becomes available you will be sent an appointment in the post.

Midwife Appointments

Arranging your First Midwife Appointment:

If you have recently found out that you are pregnant, you should arrange your first appointment with the midwife at 6-8 weeks of pregnancy. Community Midwife clinics are held at The Grange Medical Centre and Colton Mill Medical Centre.

Further Midwife Appointments

After your first Midwife appointment, your midwife will advise you when you need to arrange your next appointment.

Nursing Services (Practice and Primary Care Network Teams)

Our Nursing Team includes Advanced Nurse Practitioners, Specialist Nurses, Nurse Prescribers, Practice Nurses, Nurse Associates, Trainee Nurse Associates, Health Care Assistants, Phlebotomists and Students Nurses. We have a number of nurse led services with Senior nurses who hold additional qualifications or have specialised in the care they provide. Our nursing team works closely with a team of GP’s, Pharmacists and Specialist Nurses.

Some nursing services are not part of the GP core contract but we may provide them as part of a local scheme or shared care arrangement. You may also be referred into another service or asked to attend a local clinic.

Please speak to a member of our reception team or submit an online form to book into one of our designated nurse clinics if you have been invited, are eligible or advised by a health professional to attend.

Vaccinations

NHS vaccinations are available at the surgery as part of national vaccination programmes. Further information can be found by visiting NHS vaccinations and when to have them

NHS Travel vaccinations can only be booked after a travel assessment has been completed (Please allow 6-8 weeks prior to travel to arrange this)

Wound Care

GP practices are not commissioned to provide wound care services, however, our nursing team are trained to assess and manage simple wounds including leg ulcer management. An interim wound care service arrangement is in place with the Leeds West Yorkshire Integrated Care Board (ICB) and some GP practices. Leeds Community Healthcare and the hospitals also provide wound care services for complex/specialist wound care. You may referred you into a citywide service for your wound care or supported to self care your wounds. If you are housebound and require wound care, you will be referred into the Neighbourhood Team if you meet their criteria.

Injections

Some injections are not commissioned in Leeds for GP practices to deliver as a core service. Our nursing team are trained and can administer some injections if they have been requested by a GP, Pharmacists or Specialist Clinic under a shared care arrangement or local service.

Health Checks

Our Nursing Team are trained to offer a range of health checks depending on your medical conditions or age. New patients will be asked to complete a health questionnaire and use our self check pod to measure your BP, pulse, height and weight. The following health checks are available for those eligible.

Long Term Condition Reviews

Our Nursing Team are trained to manage patients living with a range of long term conditions. Some nurses have specialised in disease management and can prescribe. They also work closely with the GP and our pharmacists to manage your care and medications. Depending on your condition/s, you may be asked to attend an initial appointment to have your bloods, observations checks such as your blood pressure, pulse, urine, breathing test and weight along with gathering information on your lifestyle and medications. A follow up appointment will be arranged with a Senior Nurse, Pharmacist or GP to complete your review either in person or telephone.

Some patients maybe under the care of a specialist team to manage their long term condition. We may not invite you for a review if you have already attended their clinic and we have received information about your care and treatment.

Screening (Smear Tests)

Cervical screening, also known as a smear test, is a test offered to women and people with a cervix aged 25 to 64. You will either receive a national screening invitation or had an invite from the surgery if you are eligible and due your smear test.

We have evening and weekend appointments in addition to our clinics during normal opening hours.

Investigations

Some investigation tests are not commissioned in Leeds for GP practices to deliver. Our nursing team are trained and can carry out some investigations if they have been requested by a GP, Senior Nurse, Pharmacists or Specialist Clinic under a shared care arrangement or local scheme.

You may be referred or asked to attend a local Community Diagnostic Centre (CDC) for some investigations. Community Pharmacists also offer BP Monitoring Services.

Other reviews

Our nursing team may complete part of your annual review of HRT or Pill Checks. The information will be passed onto a GP or Pharmacists to complete your medication review. You can also attend a local pharmacy for your contraceptive pill.

Cervical Smear Clinics

Cervical Smear‘s are available for all women between the ages of 25 and 64.

You’ll get a letter in the post inviting you to make an appointment. 

We can only offer a smear test every 3 years for women aged 25-49 and every 5 years for women aged 49-64. 

To pre-book an appointment, contact your own GP Surgery to arrange this. 

  • Extended access smear clinics are available 8am – 2pm on Saturdays

If you are on your period, pregnant or have been pregnant within the last 12 weeks, we are unable to take smear test. This is because the result may be inadequate. 

Trans men and non-binary people who have had a total hysterectomy to remove their cervix do not need cervical screening. Trans men and non-binary people who still have a cervix should have cervical screening to help prevent cervical cancer.

Travel Health

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

To help us offer the appropriate advice, please fill out the online form before coming to see the nurse. To find out which travel vaccinations are required, you must ring the surgery 2 weeks after returning the completed travel form to see if you require an appointment to have your vaccinations.

It is important to make this initial appointment as early as possible – at least 6 -8 weeks before you travel to have enough time to book into our designated travel clinics. Not all our nurses are trained in travel health so our designated clinics can get booked up at peak holiday seasons. We sometimes need to order the vaccines in for you or you may be asked to attend a private travel clinic for any vaccines which are not available on the NHS.

  • Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
The surgery is only able to offer patients NHS vaccinations such as Diphtheria, Tetanus, Polio, Hepatitis A and Thyroid. Rabies, yellow fever, Japanese encephalitis, cholera, meningitis, tick borne encephalitis and hepatitis B will need to be given at a private clinic.

The risks below may be present in all parts of the country you are visiting.

Dengue
Dengue is a viral infection transmitted by mosquitoes which predominantly feed between dawn and dusk. It causes a flu-like illness, which can occasionally develop into a more serious life-threatening form of the disease. Serve dengue is rare in travellers.
The mosquitoes that transmit dengue are most abundant in towns, cities and surrounding areas.

Prevention
– All travellers should avoid mosquito bites particularly between dawn and dusk.
– There is no vaccination or medication to prevent dengue.

Schistosomiasis
Schistosomiasis is a parasitic infection. Schistosomiasis larvae are released from infected freshwater snails and can penetrate intact human skin following contact with contaminated freshwater. Travellers may be exposed during activities such as wading, swimming, bathing or washing clothes in freshwater streams, lakes or rivers.

Schistosomiasis infection may cause no symptoms, but early symptoms can include a rash and itchy skin (‘swimmers itch’), fever, chills, cough, or muscle aches. If not treated, it can cause serious long term health problems such as intestinal or bladder disease

Prevention
– There is no vaccine or tablets to prevent Schistosomiasis. All travellers should avoid wading, swimming, or bathing in freshwater where possible. Swimming in chlorinated water or sea water is not a risk for Schistosomiasis.
– Topical application of insect repellent before exposure to water, or towel drying after accidental exposure to Schistosomiasis are not reliable in preventing infection.
– All travellers who may have been exposed to Schistosomiasis should have a medical assessment.
 
Zika Virus
Zika virus is a viral infection transmitted by mosquitoes which predominantly feed between dusk and dawn. A small number of cases of sexual transmission of ZIKV have also been reported. Most people infected with ZIKV have no symptoms. When symptoms do occur they are usually mild and short-lived.

Prevention
– All travellers should avoid mosquito bites particularly between dawn and dusk. There is no vaccination or medication to prevent ZIKV infection. Preventing sexual transmission. Most ZIKV are acquired via mosquito bites but cases of sexual transmission of ZIKV are occasionally reported.

Food and Water Hygiene
Contaminated food and water can transmit a number of different infectious diseases such as Cholera, Hepatitis A, travellers’ Diarrhoea and Typhoid. Travellers ’Diarrhoea is particularly common in those visiting low-income countries.

Certain travellers need to take particular care as they are increased risk of complications. This includes older people, those with a weak immune system, young children and those taking medication to reduce stomach acid.

In countries with poor sanitation, it is not advisable to drink tap water or use it to clean teeth, unless it have been treated. Ice should also be avoided. Drinks serves in unopened, factory produced cans or bottles with intact seals generally can be considered safe.

Recently prepared, thoroughly cooked food that is served piping hot, fruit that can be peeled by the traveller (such as bananas and oranges), and pasteurised daisy produce such as yoghurts, milk and cheese are good options for travellers.

FAQs

The NHS provides healthcare free of charge, but there are exceptions. The doctor must establish who is funding this work and if it is not part of their NHS work, a fee may be payable. 

There is a charge for private GP letters, medical reports, signing a certificate, travel letters or insurance forms etc which are not classed as NHS work. This is inline with the British Medical Association (BMA) recommendations.
We are unable to issue letters / certificates stating patients are “fit to fly”. Assessing whether someone is fit to fly, falls outside of our area of expertise as GPs.

We are able to provide factual information about medical conditions and pregnancy if required by your airline. Please check with your airline which specific information / forms they need completed.  Please note we charge a fee for this.

For some conditions, your hospital team will be better placed than us to provide information to your airline. Pregnant women may be able to obtain the required paperwork form their obstetrics / midwife team.

The UK Civil Aviation Authority states:
‘Most airlines have medical advisors who provide advice and “clear” passengers as fit to fly… The final decision whether or not to carry a passenger is that of the airline’

Yes. GPs do not issue letters for schools, travel or insurance as part of routine NHS work. Requests for private letters or reports outside NHS work will incur a charge.

Payment will be payable at booking to cover the appointment time with a GP to carry out non NHS work and complete the documentation. You must ensure that you bring all relevant paper work to your appointment. We recommend Private Medicals are pre-booked at least 4-6 weeks. The Practice is not obliged to carry out private medicals and cannot be held responsible if we are unable to arrange an appointment within the needed time scale. In these cases, you may wish to seek a private medical service.

Self-Check Health Pod

Colton Mill and The Grange Medical Centre Self Check Pod

Both reception areas have a self check pod area which allows registered patients to take their own blood pressure, weight and height. There are a number of health questionnaires available to use and the information is automatically updated into your medical records. 

You may be advised to use these machines by a member of our reception or clinical team as part of your review. New patients are asked to use them as part of the registration process or you can use them as an opportunistic check to update your own records.

A printout of your results can be provided.

At the Pods you can:

Patients can complete Self-Check questionnaires including Contraceptive Pill Checks. The information taken will automatically download into your GP record. 

When you have completed using the pod please make reception aware.

Patients who require Contraception also have the option to visit Leeds Sexual Health, who provide a number of Contraceptive services.

NHS Health Checks

Your NHS Health Check in Leeds

Even though you might be feeling fine, Leeds is offering a free NHS Health Check which is for everyone between the ages of 40 and 74, who has not already been diagnosed with Cardiovascular Disease. You will be invited (once every five years) to have a check to assess your risk of heart disease, stroke, kidney disease and diabetes and will be given support and advice to help reduce or manage that risk.

Your NHS Health Check can help you to stay well and is free of charge.
Your NHS Health Check will take less than half an hour and involves some simple questions about your lifestyle and family medical history and measurements will be taken such as age height, weight, blood pressure. You will also have a blood test to measure your cholesterol level and your results will be given to you.

If you’re over 65, you will also be informed about the signs and symptoms of dementia, After your results have been explained, you’ll be offered personalised advice and support to help stay healthy.

By having a routine NHS Health Check every five years, you can take action early and greatly improve your chance of a longer, healthier and happier life. You may be surprised how some small, long-lasting changes to your lifestyle can make a huge difference.

How to get an NHS Health Check in Leeds

In Leeds, having an NHS Health Check is both quick and straightforward. You’ll be invited via your GP for an NHS Health Check every five years if you are between 40 and 74 years old, as long as you don’t have an existing vascular condition so please look out for your letter.

However, you can always ask your GP about receiving a Health Check too so please contact your practice for more information.

Even if you don’t qualify yet for an NHS Health Check, there are plenty of other ways to help support you to improve your health.
